Patient Online Services

You can now order your repeat medication; access information regarding your immunisations; make, amend & cancel appointments as well as viewing your allergies & adverse reactions. You will need to be registered to use EMIS Access, which can be done by providing Eyam Surgery reception staff with a form of ID.

By March 31 2016 you will also be able to view online, export or print detailed coded information held in your records.

If you would like to see this extra information please bring in some form of photo ID to reception and sign the detail coded information agreement. Detailed coded information will be available to access on or after 31st March 2016.

NHS England believes allowing patients to access such information inside and outside a surgery's opening hours will be more convenient for patients, improve safety and give patients a greater capacity for self-care.
